IMMUNIZATION REQUIREMENTS FOR HIGH SCHOOL ENTRY
- Polio vaccine - 4 doses (3 doses meet the requirement if one dose was
given on or after the 4th birthday) - DTaP/DTP/Td/Tdap – 3 doses with at least one dose of pertussis-containing
vaccine on or after the 7th birthday - MMR – 2 doses (must be given on or after the 1st birthday)
- Hepatitis B – 3 doses
- Varicella – 2 doses (or physician documented chicken pox disease history)
Requirements for Medical Exemptions of Vaccines
Beginning January 21, 2021 all new medical exemptions for school entry must be issued through CAIR-ME. Medical exemptions can only be issued by MDs or DOs licensed in California and must meet applicable Centers for Disease Control and Prevention (CDC), , and American Academy of Pediatrics (AAP) criteria. A medical exemption filed at a pre-kindergarten facility or school remains valid until the earliest of:
- When the child enrolls in the next grade span (TK/K-6th grade, 7th-12th grade)
- The expiration date specified in a temporary medical exemption
- (PDF) because the issuing physician has been subject to disciplinary action from the physician’s licensing entity.
